Class Waiver

I understand and hereby agree to the following terms when participating in yoga lessons or other events (collectively, the “Lessons”) hosted by Mint Mat Yoga Studio (“Mint Mat”).

1. When participating in the Lessons, I will follow the guidance of the instructors and performers (collectively, “Instructors”), take care for my own safety, and remain within my physical limits, under my own responsibility.
2. I am in good health and have no medical conditions that would interfere with my participation in the Lesson. In the event that such a condition exists, I will consult with a physician in advance and participate in the Lesson only with the physician’s approval.
3. If I experience any discomfort or abnormal physical condition during the Lesson, I will immediately discontinue my participation and inform the Instructor. I acknowledge that no refund will be given even if I leave the class early.
4. I agree that Mint Mat, its affiliates, or the Instructor shall not be held liable for any injury, illness, aftereffects, death, or other damages that occur during the Lesson, unless caused by their intentional or grossly negligent acts. Furthermore, I consent to receive emergency first aid from a physician or Instructor, and I will not object to the outcome of such care.
5. If I participate in the Lessons while pregnant (including maternity yoga Lessons), I will do so at my own discretion and responsibility, with prior approval from my physician. In addition, if I am pregnant or may be pregnant, I agree to inform the Instructor of such fact prior to the start of the Lesson. I further agree that Mint Mat and the Instructor shall not be held liable for any results arising from such participation.
6. If Mint Mat determines that my behavior within the facility is inappropriate or disruptive to the Lessons and Mint Mat or an Instructor gives a warning or instruction regarding such behavior, I will comply with the warning or instruction. I will also comply if I am asked to leave the facility. If I repeatedly engage in inappropriate or disruptive behavior, Mint Mat may refuse my further participation in the Lessons.
7. I understand that if I arrive more than 5 minutes late, I may not be allowed to participate in the Lesson. In such cases, I acknowledge that no refund will be issued.
8. I am responsible for managing my own personal belongings. Mint Mat will not be liable for any loss, theft, or damage to my personal property.

*The personal information you provide will be used solely for the purpose of managing the Lessons and for emergency contact. It will be appropriately managed and will not be shared with third parties without your prior consent, except as permitted or required by law.
